Keep Your School Nurse Informed
Student health information forms must be completed by parents each school year. Please drop off completed forms at the school office.
Keep the school nurse informed on your child’s health status. If there are any changes to your child’s health such as surgeries, medications, illnesses/diseases, new diagnoses, or other conditions that may affect your child’s school performance, please notify the school nurse.
